2. Study protocol(3 copies)

3. Facing sheet duly signed by Academic supervisor, co-supervisors and Head of Department(1 copy)

4. Subject information and Informed consent (English Malayalam)(13 copies)

5. Study Proforma/questionnaire (13 copies)

6. Copy of receipt of Ethical Committee submission fees according to guidelines given in the SOP(1copy) at accounts department

7. A brief CV with contact details (1copy)

8. Ethics committee archive copy should be properly filled and all documents should be mailed to [email protected] and the name of the file should start with the researcher’s name.
